Transitional Care Center: Post Discharge Med
The High Risk Center at the University of Maryland St. Joseph Medical Group is tailor made for patients at high risk for medical readmissions to hospitals. We provide a comprehensive care and follow up for hospital post discharged patients with a physician evaluation and follow up, pharmacist counseling, and a care manager education for our high risk patients. Patients seen by our Transitional Care Center will receive attentive follow up and reminders, as well as assistance in finding the appropriate primary care or specialist for long term continuity of care. Our care manager also provide necessary follow up guidance for financial and lifestyle assistance to improve medical care management.
