Transitional Care Center
The Transitional Care Center provides outpatient support to patients who have recently been discharged from the hospital and need assistance with medical or behavioral health concerns. The Center seeks to reduce readmissions to the hospital, and ensure that patients get the care they need in the community.

Post Discharge Center
The Post Discharge Center supports patients at high risk for medical readmission to the hospital. We provide comprehensive care for recently discharged patients, including physician evaluation and follow up, pharmacist counseling, and education for our high risk patients. Patients seen by our Transitional Care Center will receive attentive follow up and reminders, as well as assistance in finding the appropriate primary care or specialist for long term continuity of care. Our care manager also provides necessary follow up guidance for financial and lifestyle assistance to improve medical care management.

Behavioral Health Clinic
The Behavioral Health Clinic at the University of Maryland St. Joseph Medical Group Transitional Care Center is a rapidly accessible mental health clinic that serves patients who were recently discharged from the hospital or emergency department as well as patients referred by ambulatory providers in the UM St. Joseph Medical Group. The clinic serves adult patients with a primary psychiatric disorder who have been referred by a UM St. Joseph provider.
Special areas of focus are mood disorders, women's mental health, and people experiencing both physical and mental health problems.
Services offered include psychiatric diagnostic evaluations, psychiatric medication management, individual psychotherapy, group psychotherapy, and mental healthcare coordination services. Modalities of therapy include cognitive-behavioral therapy (CBT), dialectical-behavioral (DBT), supportive therapy, and motivational interviewing.
Previously called "The Bridge Clinic," the Behavioral Health Clinic provides transitional support for patients for up to 90 days. The interdisciplinary care team crafts an individualized aftercare plan, which includes referral to ongoing mental health services if needed.
